Medicines and durable medical equipment to be provided as needed and per availability. Follow-up and resources to be provided for unique cases. Number of participants: 100+ plus including Barangay’s participants and provincial health office participants
Number of Persons to be served: 200+ for rehabilitation services; 300+ for dental basics only, and 300+ others.
Population Served: Adult to elderly who are in need of basic dental and rehabilitation interventions (i.e. strokes, cardiac, diabetic, and others). Pediatric to early adulthood who are in need of dental service, and rehabilitation intervention (strokes, cerebral palsy, trauma, visual impairments and other special needs). Public healthcare (volunteers)/Caregivers/family members are encouraged to participate to carryout home/school programs for recipients to benefit better in any of the services provided.
Barangay covered for this project: Paco, Colalo, and Cabiten

* Almost all information above especially the one in asterisk is not final as it may need modification for details and assignments. We will inform for any changes above to everyone involved.
* It’s currently being discussed whether we will do a 3-day event one for each Brgy or divide into 3 different teams for each Brgy in one-2 day; in exception of Rehab/Seminar service as I may need to do 3-day one for each Brgy since training is important. We are considering the best way possible to serve the community.

Proposal Narrative
Intro:
I, Anna Lyn Caytap, hereby to ask for support and guidance to promote wellbeing of individuals in need in the community named above. I have been working with people with disabilities (all ages) for 10 years. I am currently a Licensed Occupational Therapist in San Diego, California. I received my undergrad in Communicative Disorders. Prior to being an OT, I worked in the field of Speech and Language Pathology (SLP) as a technology specialist working in communication boards, computer adaptation, and home modification for people with disability. I also worked with pediatrics as an SLP Assistant prior. I now work at a Rehab Center and also float to the acute & ICU floors. I also work with pediatric as well as home health.
Currently, I am collaborating with dedicated people from the Mankayan, Benguet local officials, health workers, family members, friends, colleagues, Lepanto High School Alumni, Batang Lepanto Mines Corp., and current LNHS students.
In addition, I will provide 2-day seminar for health workers and caregivers prior and after this event. This is in conjunction to the rehabilitation education and training that is a part of this mission. This is to maximize training to carry out basic skills in helping individuals in need.
Mission Statement:
In our desire to help our community, this project came about and materialized with everyone’s support and dedication to participate. Each participant is equipped with different expertise from the medical field, education, engineering, caregivers, contractors, and other fields. This is to acknowledge and share the blessings we all receive and to provide a sincere service to a community in need. The joint project’s goal is to provide health related education, basic medical services, and rehabilitation services to improve quality of life and wellbeing of Paco, Colalo, and Cabiten. We will collaborate as one team to plan and implement strategies to meet our goal. The second goal is to provide transition to government aid for continuity of care for those cases that are unique or shows promising results. I hope for a continuation of project like this to other neighboring communities in the future.
It is my hope that each participant will maintain respect and consider cultural sensitivity and the nature of each individual’s belief.
Area and Population Served: This year’s joint medical mission project will serve individuals residing in Brngys Paco, Colalo, and Cabiten. We will utilize government facilities such as school buildings and sport open grounds for the safely of all involved. An estimated 500+ individuals for the two days event will be overseen by participants according to each expertise. We hope to serve individuals from pediatric to geriatric population. Adult to elderly who are in need of basic dental, pregnancy consultations, and/or rehabilitation interventions (i.e. strokes, cardiac, diabetic, and others). Pediatric to early adulthood who are in need of dental service, and rehabilitation intervention (strokes, cerebral palsy, trauma, visual impairments and other special needs). Public healthcare (volunteers)/Caregivers/family members are encouraged to participate to carryout home/school programs for recipients to benefit better in any of the services provided.
Project Plan:
An initial survey will be collected in collaboration with the Brgy health workers in charge. Dates of the event will be announced via the Brgy health workers. Recipients will need to sign up and screened to assign which areas of focus (see below) they may benefit from. Assigned team will coordinate with Mayor Luspian, Dr. Piok and the provincial health office regarding logistics, obtaining census, and contact persons. Medicines will depend on recipients’ basis. We will also provide caregiver education for them to have a better understanding of the issues surrounding the recipient’s health or disability.
I. Areas of focus… (approximate numbers)
1. Rehabilitation service – (at least 300+) overseen by therapist
- Qualified professionals will coordinate to screen/assess needs of service
- Education and essential skills training will be provided to recipients and family/caregivers to have skills necessary to carry out home exercise programs and home environmental modifications
- * Extended service – follow up and progress results will be followed by trained Brgy health workers (those who attended the workshops/seminars)
2. Basic Dental Procedure– (at least 300+) to be managed by qualified dentist and assistants
- Basic dental procedures only, no major surgical procedures
- Will provide resources for major surgical procedures
3. Pregnancy consultation – (at least 150+) overseen by qualified professionals that will inform/educate mothers-to-be regarding existing concerns, nutrition, and other issues involved.
4. Other education services – (at least 300+) such as DM, Infection disease, HTN, and others. Medicines will be dispersed based on recipients’ needs and medicines availability
- Healthcare workers for each Brngy are encouraged to attend and be a part of this group
II. Other services… will collaborate with the Barangay officials’ cooperation.
1. Transportation needs– Assigned team will coordinate with major transportation corporation to participate in this area. Transportation may play a role on attendance.
- Jeepneys and buses are essential to provide transportation to all recipients especially for those who are coming from far distances
- Vans may need to be provided for those who are more involved (mobility wise)
2. Food Preparation – assigned team will be responsible for…
- Food, Paper, cups, and utensils purchase purchasing
- Locate appropriate place to cook food – easy access
- Materials needed: cooking utensils, pots, and pans
- Will make sure that there is available water supply for cooking and cleaning
- Will assign team to supply drinking water to participants and recipients
3. Safety measures – Assigned team will collaborate with the Barangay volunteers to insure everyone’s safety
- Utilize unarmed security volunteers; Plan safety routes; bathroom access
- * Walki-talkies and other means of communication will be used if available
4. Surveyors – Assigned team will collect data before/after each service.
- Surveyor’s fluency in a dialect or language will be considered
- Copy of a survey form will be handed to each recipients and/or family members to fill out with assistance as needed
- To avoid affecting the survey outcome, a surveyor has to read the content of the form verbatim
5. Non-Lepanto residence participants room & board – Collaborate with the Brngy officials. Assigned team will insure that participants who are not Lepanto residence will have a place to stay in.
- If need be, we will acquire permission to use rooms in elementary school to board participants
- Or will assign Lepanto residence sponsors to take in participants
6. Prayers – Priests/religious leaders who may want to lead prayers before and after the service
- We will invite religious leaders within the community to participate
Expected Project Outcomes:
It is our hope that we can serve most, if not all, recipients that signed up and shows up on the day of the event. During the event, measurement tools and checklists for both recipients and participants will be in place to record outcomes to help us improve our planning and sufficiency for future activities we will carry out.
Surveys for success and improvement includes but not limited to:
1. Number of attendees and number of age population (overall total including served and not served)
2. Number of recipients in each category will be recorded
3. Number of participants and volunteers will be recorded
4. Number of unique cases that requires follow up will be recorded
5. Number of recipients that requires Van or Bus/Jeepney transportations will be recorded separately
6. Participants survey to measure and improve the quality of our service
7. Recipients’ survey to measure and improve the quality of our service
We will continue to follow the outcomes of each identified unique cases to further our understanding of the community’s needs. Following unique cases is imperative to an individual’s hope of gaining some quality of life and burden of care. We hope to endlessly collaborate with the community and its alumni to establish a healthier life and wellbeing of the community.
Government Assist Requested: Continued collaboration with Brngy officials/healthcare staff. We hope for cooperation and manpower for smoother planning and organization such as use of the government facilities and other logistics. We hope that the transition of referred unique medical cases (made aware of during the day of medical mission) to a government owned clinic/hospitals for further tests and medical care will be less rigid for the family.
Sources of Funding: Solicitation and fundraising activities. Sponsorship. Assigned teams will supervise charity projects. (Possible government grant/s in the future.)
Budget: Approximately P270,000.
Plans for Future Funding: Any remaining funds will be used for the follow ups that some recipients may need for additional medical expenses for a short period of time.
